This gray-white plastic punch card is indented at the two top corners and notched at three places along the right side. It has no holes (it is "undrilled"). The card is numbered across the top 00, 10, 20, 30, . . ., 90. It has a translucent green plastic backing.
A mark on the lower right corner of the card reads: TERMATREX Products, Inc. (/) Lower Level (/) 26 North Summit Avenue (/) Gaithersburg, Maruland 20760 (/) 301-840-1790. A mark on the left bottom corner reads; TERMATREX SYSTEMS (/) RANDOM NUMERIC CARD (/) RE-ORDER No. RN-TC-1000/.
